DISC PROTRUSION

PRACTICE POINT :

Bulging intervertebral discs are no more abnormal than skin-wrinkles.

The cause of most low back pain is unknown.

Although an accident may cause a degenerate intervertebral disc to become symptomatic for the first time, objective evidence of causation is rarely possible.


TRAUMATIC CORONARY

PRACTICE POINT:

Driver heart-attack very rarely causes serious-injury traffic accidents but is often fatal.

Recent technical advances have improved the POTENTIAL ability to diagnose co-existent heart-attack and serious bodily injury, both before and after death.


MISSED APPENDICITIS

PRACTICE POINT:

In missed appendicitis, look for
  1. a rectal examination
  2. administration of narcotics
  3. diagnosis of gastroenteritis
  4. clear discharge instructions

PLACENTAL FRAGMENTS

PRACTICE POINT:

In cases involving Delayed Post Partum Hemorrhage, administering Oxytocin after delivery may have masked signs that placental fragments remained.

Trauma from curettage (scraping) of the lining of the uterus in pregnancy, or during the first month after delivery, preceded Asherman's Syndrome in 9 out of every 10 cases.


NEGLIGENT FISTULAE

PRACTICE POINT:

In the presence of risk factors for urinary fistula during gynecological surgery, look for
  1. special precautions to protect bladder and ureters
  2. filling of the bladder by catheter to identify damage early.
Postoperative indicators of damage to the bladder:
  1. severe abdominal pain
  2. abdominal distension
  3. slow return of bowel function
  4. blood in the urine
  5. irritability of the bladder
